Information we collect

Information you send us through the contact form. If you use the contact form on this site, we collect the information you enter: your name, your email address, your phone number (if you provide it), and the message you write. Please keep your message general — do not include detailed health or medical information in the form. This website's contact form is not a secure medical portal and is not monitored for emergencies. If you have a medical emergency, call 911.

Information collected automatically when you visit. This website is hosted by Google Sites. When you visit, Google may collect technical information such as your IP address, browser type, and the pages you visit, as described in Google's Privacy Policy. 

Information we do not collect through this site. The atrial fibrillation (AF) calculator on this site does not ask for your name or contact information, and it does not store the values you enter. Your calculator inputs are used in your own browser to produce the results you see.

How we use your information

We use contact form information only to read and respond to your inquiry, and to maintain ordinary business records of that correspondence. We do not use it for marketing, and we do not sell personal information.

How information is shared

We do not sell your personal information, and we do not share it with third parties for their own marketing. Information you submit is stored through our Google Workspace account (Google Forms and Sheets) and emailed to us so we can respond. We may also disclose information if required by law or to protect the safety and rights of our practice and others.

How long we keep information

We keep contact form messages only as long as needed to respond to your inquiry and to maintain business records, and then we delete them.

Your rights

If you are a Washington resident, you may have rights under Washington law, including the right to know what personal information we hold about you, to ask us to delete it, and to withdraw consent for its collection, subject to limits in the law (for example, information we must keep for legal or healthcare record-keeping reasons). To make a request, email andrew@collaborativecarewa.com and tell us what you are asking for. We will respond as the law requires.

Children's privacy

This website is not directed to children under 13, and we do not knowingly collect personal information from children. Please do not submit a child's personal information through the contact form.

Security

We take reasonable steps to protect the information submitted through this site. However, no method of transmission over the internet is completely secure, and we cannot guarantee absolute security.

Changes to this policy

We may update this policy from time to time. When we do, we will update the "Last updated" date at the top of this page.
