Care that comes to you — from a clinician who knows you.
If you live in one of the nursing homes or assisted living facilities we serve in King County, Collaborative Care brings your medical care to you. No arranging rides to outside appointments for every concern — your clinician comes to where you live.
• The same clinician, visit after visit. Andrew Grossman, ARNP, years of experience in nursing homes and assisted living. He'll know your history, your medications, and what matters to you — not just your chart.
• Care in your own room. Visits happen where you live, coordinated with the facility's nursing staff.
• Families stay in the loop. We believe families are part of the care team, especially when decisions get hard.
• Wound care — Wounds that heal slowly get a consistent plan and close follow-up from a clinician certified in wound care.
• Foot care — Regular attention to foot problems, especially important if you have diabetes or circulation problems. Timely referral to podiatry when needed.
• Palliative care — Relief from pain and other symptoms, and unhurried conversations about goals and wishes, from a clinician certified in hospice & palliative care for over 10 years.
• Primary care — Ongoing management of your health conditions and medications, with regular review of whether every medicine still earns its place.
• When something changes suddenly — Years of urgent care experience means prompt, experienced evaluation when you're unwell.
