A clinician partner who knows your residents — and your building.
Collaborative Care PLLC, led by Andrew Grossman, ARNP, partners with nursing homes and assisted living facilities in King County to provide on-site medical care for residents — without your staff coordinating outside appointments for every need.
Why facilities work with us
• Experience that fits your setting. Working in nursing homes since 2008. Andrew understands facility workflows, documentation, and the realities of caring for medically complex residents.
• Certified wound care. Certified in wound care — consistent assessment, treatment plans, and follow-up for pressure injuries and other chronic wounds, right in the facility.
• Palliative care expertise. Certified in hospice & palliative care — symptom management, goals-of-care conversations with residents and families, and guidance when hospice may be appropriate.
• Primary and urgent care skills. Urgent care practice means changes in condition get prompt, experienced evaluation — and clear judgment about what can be managed in place and what needs a higher level of care.
• Continuity. The same clinician, visit after visit — residents, families, and staff aren't starting over each time.
